- A statement that you have a good faith belief that the content was removed or disabled as a result of mistake or a misidentification of the content.
- Your name, address, telephone number, and e-mail address.
- A statement that you consent to the jurisdiction of the federal court in the district where you live (if you are in the United States), or your consent to the jurisdiction of a federal court in the district where our company is located (if you are not in the United States).
- A statement that you will accept service of process from the person who provided notification of the alleged infringement.

Repeat Infringers
It is our policy to terminate the accounts of repeat infringers when appropriate. We will terminate the accounts of users that are determined by us to be repeat infringers. If you repeatedly download content that infringes upon the copyrights of others, your access to our service may also be terminated.
